A collaborative study from the University of Gothenburg and Chalmers University of Technology, published in Acta Dermato-Venereologica this week, applied AI models to Swedish national registry data covering 6,036,186 adults to predict melanoma risk up to five years before diagnosis. Over the five-year study period, 38,582 individuals (0.64 percent of the cohort) were diagnosed with melanoma, which gave the model a real-world validation set at population scale. The headline result: the most advanced AI model correctly distinguished between people who went on to develop melanoma and those who did not in approximately 73 percent of cases, versus a 64 percent baseline using only age and sex. That is a nine-percentage-point absolute improvement, which is the kind of gap that matters when you are making screening-resource allocation decisions across millions of people.

The features that made the model work are worth flagging because they are not what most medical-imaging AI pieces describe. This is structured-data prediction from medical records: prior diagnoses, prescribed medications, sociodemographic attributes. There is no image input. The model is essentially a large-scale tabular classifier trained on electronic health record data, which is a different technical problem from the image-based skin-cancer detection work that has dominated the field for the past decade. The concrete clinical implication reported in the study is that the model can identify smaller high-risk groups who face a 33 percent risk of developing melanoma within five years. That 33 percent concentration on a small cohort is high enough to justify intensive follow-up screening that would be economically unjustified across the general population. Dermatology screening is cost-constrained in most national health systems; if you can shrink the monitored cohort by an order of magnitude while capturing a substantial share of cases, the economics tilt toward implementation.

This fits a pattern across medical AI work this year that is worth naming. Scientific AI is becoming operationally useful through prediction-plus-selective-intervention rather than through sweeping screening replacement. Similar dynamics appear elsewhere: AI-enhanced low-field MRI does not replace high-field MRI, it lets cheaper hardware produce diagnostic-grade output for a subset of cases. Google's MoGen does not replace expert neuron tracing, it augments the data-labeling pipeline. This melanoma study does not propose universal AI-driven screening, it proposes AI-driven high-risk-cohort identification. The common thread is that AI in health care is scaling not as a general replacement for medical judgment, but as a prioritization layer that makes scarce clinical attention more efficient. Builders working in health-tech should study this framing: the question is not "can AI match a dermatologist," it is "can AI identify the patients a dermatologist should see first, cheaply enough to deploy at national scale." Those are very different product problems.

For anyone building medical-AI prediction tools, three concrete observations. First, tabular-EHR prediction is undervalued compared to imaging AI in public discourse, and the Swedish study is one of many recent examples where structured medical records outperform what you would expect for a given task. If your health-AI product design assumed imaging was the only viable input, that assumption is probably wrong. Second, the metric that matters is not headline accuracy, it is the precision of the high-risk cohort identification and the concentration factor (how much you can shrink the screening population while retaining sensitivity). In the Swedish study that number is the 33 percent risk concentration on the small high-risk subgroup; in your product, it is the equivalent concentration on whatever the equivalent actionable cohort is. Third, the regulatory and deployment path for a prioritization tool is substantially easier than the path for a diagnostic replacement. Position your product as augmenting scarce clinical attention, not substituting for it, and the approval conversation gets meaningfully shorter.
